Do you ever order toast, but it comes burnt? Do you eat it or send it back? How often does this happen to you? Yesterday it happened to me three times in one day. What can we learn from these quality failures?
“We’re babysitting our daughter’s dog; she’s only 10 months old and she needs some exercise, so we took her to the Hobnob daycare center. When we got there, the woman up front, kind of with a gruff tone, said “Well, you need to make a reservation.” My wife Shirley said “Well, our daughter made a reservation.” “Well, I don’t see it here,” you know?

Did your quality instructors teach you to gather a team and brainstorm a problem to solve? That’s terribly bad advice. Use data to pinpoint the problem. Then, and only then do you know who should be on the root cause analysis.
I don’t know about you, but when I first got my quality training they said, “Get a team together and brainstorm a problem to solve and then go start trying to find the data, define the data, measure the data,” or something else. Now that’s the dumbest damn advice I ever got in quality training. I’ve looked at a lot of different training packages over the years and they still have all that same stupid stuff in there.

How do you know when your countermeasures are working? Nothing bad happens. But it’s often difficult to notice unless you look back into past. We all have absence blindness – we can’t see what isn’t there.

“Now, I don’t know about you, but I spent the last year and a half wearing a mask, washing my hands and staying out of crowds. Surprisingly enough, I didn’t get COVID and my wife and I went to the Grand Canyon and Yellowstone and went to Hawaii on vacation, but we didn’t catch COVID.

I’m here on Ka’anapali Beach in Maui taking a little vacation, but I was thinking about this: “I’ve talked to a lot of people and they seem to think that that they have all these meetings where they fix special causes and they think they’re doing Quality Improvement. They’re not. If you’re just fixing special causes, that’s part of the Fix-It Factory. You want to become part of the Prevention Factory.

If you think your role in Quality Improvement is merely to prevent special cause variation, maybe it’s time to go “Upstream” with your thinking.

“I just finished reading Dan Heath’s new book “Upstream,” and for those of you in Quality Improvement you would see the pattern in here instantly as just continuous improvement: get some data, figure out how to fix things and then fix them. He’s tackling bigger things like… “How do we reduce murders of abused women?” or “How do we reduce dropout rates in schools?”
